Chargeback when customer was refunded?

This question is about order 111-3082408-5985846 from last month. We shipped this customer what he ordered on time via FedEx. As recommended by Amazon for pricier items, it was sent signature required. FedEx, attempted delivery four times but nobody was available to sign for the package and so it was returned to us. When we first learned it was on its way back to us, we issued the customer a full refund. However, two days later, the customer filed a credit card dispute even though it had been refunded. We presented our case and was told, "sorry- this is a chargeback and it stands and will affect your ODR". We opened a case to ask what it was we did wrong. The case was transferred weeks ago with no response. Anybody have any ideas as to what we can do next to better understand what happened or better still have this chargeback removed?
