FBA customers being refunds without return of product

How can Amazon keep issuing refunds to all customers without any investigations? This is absolutely absurd. We had reasons like refused delivery and did not receive but without any items going back into our inventory. Amazon keeps taking our FBA fees, however offer no protection in return. Then what is the fee for?
