Split Shipments Issue - Another Glitch
This whole new shipping setup for FBA has been nothing but a mess. Either the system doesn't work for hours on numerous days or there are major glitches.
Today is a first. It split to 3 locations. Fine. Well it will not give a UPS shipping price or label for one of the three. Regardless how many times we refresh. I caught it on the first attempt where two locations printed the FBA and UPS label and the third just a FBA label.
All proper boxes are checked. So now we can not send anything in, again, on a Friday.

We have EXACTLY the same issue today - I've opened a case and of course the first response was boiler plate instructions how to set up Ship to FBA. I replied asking then to look at the screen shot they requested.
If nothing else with your post I at least learned it's not my problem but it is Amazon's issue

just got the "clear your cache" response:
It is possible, that it could be a browser issue. In these cases, you can do the following
1. Update your browser
2. Clear cache and cookies

I had this exact issue last week when I tried sending in 7 boxes with UPS SPD that got split into 5 shipments. I got FBA labels for all the boxes but one UPS label never generated. I tried a few times with different workflows and had the same issue every time.
I spoke to seller support for an hour, sent in screen shots...got a response from a support ticket that everything looks fine and try again...but of course I still ran into the same problem. Eventually I lowered my shipment to 6 boxes split into 5 shipments and then I got proper labels for everything.
I agree the system is flawed and unfortunately Amazon doesn't seem to think there's any problems here...

New system keeps forcing us to pay the placement fees even though we are shipping 4 to 5 identical boxes with identical Qty and SKUs. Then on top of this I am getting nasty messages from Amazon telling me I am not doing good at keeping FBA stock as we are moving as much as we can over to FBM as FBA inventory runs out. UGH!!!!!!

We have had the exact same problem you described. Clearing the cache did not work. Opened a case with no helpful response yet. @Topher_Amazon are you able to look at my case? Case ID 15418347891
